NORTHUMBERLAND HOSPICE SERVICES - FACILITATED
INTEGRATION
The Central East
LHIN is partnering with Hospice Northumberland Lakeshore and
Palliative Care Campbellford on a facilitated integration that will
see hospice services, currently provided by these two agencies,
integrated into the group of services provided by Community Care
Northumberland. An Integration Planning Team, comprised of
representatives from Hospice Northumberland Lakeshore, Palliative
Care Campbellford, Community Care Northumberland and the
LHIN has just begun meeting and will, over the summer,
develop an implementation plan to be reviewed by their respective
boards. The goal is to have the plan approved in early fall and to
then immediately begin implementing the new integrated model for
hospice services in Northumberland.
